Box Breathing, Physiological Sighs and Coherent Breathing, Compared

Three famous techniques, one honest comparison — what each actually does, what the head-to-head evidence shows, and which to use when.

Box breathing, the physiological sigh and coherent breathing are the three most-cited breathing techniques in the calm-down literature — and they are not interchangeable. They differ in mechanism, in time-to-effect, and in what the head-to-head research actually shows. Short version: the sigh calms fastest, coherent breathing builds the best baseline, box breathing steadies attention — and the famous month-long comparison found breathwork in general outperforming mindfulness meditation for mood, with the exhale-weighted sigh leading (Balban et al., 2023).

The mechanisms, briefly

All three work the same underlying levers — respiratory rate, the inhale/exhale balance of autonomic tone (heart rate rises slightly on inhales, falls on exhales), CO₂ tolerance, and attention — but they weight the levers differently.

Physiological sigh (double nasal inhale, long mouth exhale): maximises the exhale lever. The second inhale pops open collapsed alveoli, enabling a fuller offload and a longer, heart-slowing exhale. It is the body's own reset — you already do it involuntarily before sleep and after crying.

Coherent breathing (~5–6 breaths per minute, even in/out): tunes the whole system to the resonance frequency where heart-rate oscillations are largest, maximising heart-rate variability — the pattern at the centre of HRV-biofeedback research and reviews of slow-breathing physiology (Zaccaro et al., 2018).

Box breathing (equal in–hold–out–hold, classically 4-4-4-4): moderate rate-slowing plus two holds that add a light attentional task and mild CO₂ exposure. Less exhale-weighted than the others — its distinctive product is composure: a busy mind given a track to run on.

What the comparison study showed

The Stanford trial worth knowing ran four groups for one month, five minutes daily: cyclic sighing (exhale-weighted), box breathing, cyclic hyperventilation, and mindfulness meditation. All improved; the breathwork groups improved mood more than meditation; and cyclic sighing led — including a reduction in resting respiratory rate, a plausible marker of a genuinely down-shifted baseline (Balban et al., 2023). Honest caveats: one study, modest sample, self-reported mood — a strong signal, not a settled hierarchy. But it agrees with the mechanism: if calming is the goal, weight the exhale.

Inhales rev, exhales brake. Every calming technique that works is, at bottom, a clever way of braking more than it revs.

The decision guide

  • Acute stress, sixty seconds available → one to three physiological sighs. Fastest onset by mechanism and by trial.

  • Daily practice slot, five to twenty minutes → coherent breathing. Best baseline-builder; the natural core of a morning practice and the workhorse of any longer programme.

  • Performance pressure — composure with alertness → box breathing. The holds keep attention occupied without over-sedating; shrink to 3-3-3-3 if fours strain.

  • Settling before inner work → sighs, then two minutes of extended exhale — the standard preamble to visualisation, because imagery follows state.

The meta-skill

The comparison matters less than the competence underneath it: knowing that your breath is a control surface, and reaching for it on purpose. Whichever pattern becomes yours, the practised move is identical — notice the state, choose the lever, spend the sixty seconds. Do that daily for a season and the techniques begin to blur into something simpler: a system that trusts you to drive it.
